Care Team

The LCUUF CARE TEAM serves as a point of contact for any health, personal or emotional concerns our members may be experiencing. The team will respond and communicate appropriately to concerns in a timely fashion. Rev. Matt will be contacted immediately if that is appropriate. As each person in each situation is unique, each response will be unique as well.
The Emergency Contact Information Form is a confidential form, which will help the team in contacting your primary physician or loved ones in case of an emergency. We encourage all members to have this information on file. The form is submitted automatically to the care team’s Emergency Data Manager. It can be filled out by clicking the link below:

What We Do:
- Maintain a confidential emergency contact form on each member.
- Provide compassionate listening.
- Provide hospital/home/care home visits
- Organize short term meal trains
- Send emails and birthday cards as appropriate
- Provide referral to pastoral or other counseling.
- Provide information for resources outside of LCUUF.
- Light a special candle on the altar at LCUUF each Sunday in recognition and support of unspoken concerns and sorrows.

Once your email or call has been received, a member of the team will attend to your questions or concerns. In some cases, we may refer you to others who can better help you.
In case of a life threatening emergency: go to the nearest emergency room (see below). DO NOT call the police as they do not handle health emergencies or 911 as it is linked to Guadalajara and not to local first responders, so is not recommended.
